it's normal to have proper ranks when starting salah, but in the middle of salah you should have better things to do like concentrating than checking your neighbour's feet

it is also normal for the ranks to be a bit disorganised during salah

exactly.. prayer shouldnt be about you worry where your feet are or if your shoulders are still against the person next to you. Yes, at the start you should make sure the line is straight and position yourself correctly but once you have started, your sole concentration should be in the conversation will Allah (SWT) and not on your own position of your feet etc or what the person next to you is doing.
