What exactly does this mean?

If people were not caught up in the dunya, would medicinal advances have made it so far?

I mean don't we need to be involved in the dunya to some extent?

We are to excel in both, the Dunya and the hereafter, however I think when they say not to get SO overwhelmed where you forget that this dunya is just a test. You shouldn't also just forget the dunya completely and go into hibernation.

Often we forget what we get is from him, Allah (swt) and were not grateful. We get too caught up in our dreams and are constantly trying to reach higher. I mean I suppose its fine and good to always aim higher and to always get better. But we forget were not going to take any of it with us.

Don't get caught up in dunya doesn't mean don't do nothing at all to benefit yourself in this world it just means don't get too involved.
Its like when you have an exam at school teachers tell you to revise you obviously u don't revise 24/7 but that becomes your priority.
Like that worshipping Allah(swt) it supposed to be prioity in this world.
advancing in fields such as medicine is also a form of worship as you are benefiting other creations of Allah.

Unfortunately, Dunya is temporary. In fact, it is veeeery short.

That's good news for the believers, who look forward to the everlasting joy of akhira(Hereafter).

Indeed. Worship in Islam is two types, the ritual (praying, fasting, etc) and then also all act of goodness that are encouraged in Islam. Caught up in the dunya would be more about being hungup about wealth, power, etc. There is a popular dua in the Qur’an where we ask Allah (swt) for the best in this life and the next.

Al-Salamu Alaykum:

The best of the youth, Sayyiduna Ali ibn Ali Talib said: "Zuhd (or doing without this world) is not that created things be in your hand but that they don't reside in your heart"

Any deed that has attached to it a Niyyah for Allah most has reward in the next world. Anything of the next World is never considered Dunya. However,

Prayingor reciting Quran to show off in front of people is a next wordly act with a Wordly intention attached to it and will not be rewarded in the next world and Allah knows best.
