in prayer, six items are fardh.

1) to say takbir after intent.
2) to stand.
3) to recite sum surah or verse from qur'an.
4) to bow in ruku
5) to perform both sajdahs.
6) to sit for the time required for recitation of attahiyat.

if the fardh acts are missed out of prayer then the salah is not valid.

sum acts are wajib.

1. recitation of surah fatihah and adding sum surah with it.
2. to perform each fardh properly and in order.
3. to recite surah fatiha and other surah standing.
4. to bow properly in ruku and do full sajdah.
5. to sit and recite attahiyat.
6. to recite dua e qunnut in witr prayer.
7. to turn face ryt and left wen givn salaam.
8. to perform all acts slowly and with patience.

the rest is either sunnah or mustahab. if the wajib acts are missed then either the salah shud be repeate or sajdah e sawh shud be made.

The above is the hanafi madhab ruling. :)

The Prophet prayed Dhur or Asr and said taslim after two rakah. People left the masjid in a hurry saying the salah has been shortened. The Prophet stood leaning against a pillar in the masjid as if he was angry. A man stood up and said ‘Messenger of Allah have you forgotten or has the Salah been shortened? The Prophet replied ‘I haven’t forgotten nor has the Salah been shortened’. The man said ‘then you have forgotten’. The Prophet asked the companions ‘is what he said true?’ They said ‘yes’. So he went forward completed the remaining of his Salah then said taslim and then he did two prostrations and said taslim. (Al-Bukhari and Muslim)

The Prophet prayed five rakah for Dhur. He was asked ‘has the Salah been increased?’ He said ‘what is that? They said ‘you prayed five rakah’. He did two prostrations after he had said taslim. (6)

Deliberately missing any rukn or wajib act in salah nullifies it
Without the first takbir the Salah is not begun
If you forget one of the arkan you must go back to it or ignore the whole rakah and do two prostrations after taslim

Sujud as-sahu has to be done before taslim for missing a wajib act
The prophet prayed Dhur he stood up after the first two rakah without sitting. People stood up with him. When he completed his Salah and people were waiting for him to say taslim he said takbir and prostrated twice before taslim and then said taslim. (Al-Bukhari)

Doubts are not considered in three cases
If it is only a thought
If you suffer from it often
If it occurs after the completion of the Ibadah unless you are certain that something happened

You are doubtful of whether you made a mistake or not and you cannot decide which is more likely
In this case you go by certainty and that is going by the least amount
‘If one of you has doubt in his Salah and doesn’t know how many he has prayed, three or four, he should get rid of the doubt and go by what he is certain of then he should prostrate twice before taslim…’ (Muslim)

Sujud as-sahu is only done once even if there is more than one mistake
If sujud is required for before and after taslim then it is done once before it
Takbir is said for each sajdah
Tashahhud is not required after sujud as-sahu
