Travel Prayer Rulings.

I lost my book so I can't look it up, but the prayer rulings change when you travel beyond a certain distance and if you are there for a certain number of days or under.

Does anyone know the rulings e.g.
how far do you have to travel for the rulings to take place
how many days do you have to be there to invalidate the travel rulings
how many rakaat do you pray for each prayer, i think its 2 farz for everything?

If you travel more than 48 miles for less than two weeks you pray Qasr salah.

For zuhr, asr, esha you pray 2 fardh instead of 4, the rest stays the same.
