Sajdatus-Sahw

I keep reading that you have to do this after or before tasleem, depend whether you have doubts or you were certain that youd did something wrong. Basicly this is confused me.

Can anyone just tell me when do I have to do Sajdatus-Sahw in the following scanerios(because I think I'm more likely to do a mistake here)

1. When I know that I missed the middle tashheed.

2. when I doubt whether I missed the middle tashheed.

3. When I know I missed the second bowing down in sujood.

4. when having doubts of number 3.

5. When I don't remember what raka I am doing at the moment.

6. When I have doubts if I did the ruckoo when doing sujood.

And can you please tell whether I should do the Sajdatus-Sahw BEFORE or AFTER the tasleem.

As far as I am aware, you do the prostration of forgetfullness when you are fully aware you have forgotten something in your salah. For doubts in salah, I think you are meant to conclude that you did perform it but can't remember?

Is tasleem saying salaam at the end of your salah? As far as I know when youare in your last sitting, and you reach the end, you say salaam to one side only and thereafter prostrate and complete your last sitting again (tashahud, durood etc) and then salaam to both sides.
